Abstract
Background: Biliary tract carcinoma (BTC) is an aggressive malignancy with poor prognosis and limited therapeutic options. Claudin 18.2 (CLDN18.2) has recently emerged as a promising therapeutic target; however, data regarding its expression in BTC remain limited.
Objectives: To determine the prevalence of CLDN18.2 expression and investigate its association with clinicopathological characteristics in patients with biliary tract carcinoma.
Materials and Methods: A retrospective, descriptive study was conducted on 83 histologically confirmed BTC cases (53 surgical resection specimens and 30 core needle biopsies), including gallbladder carcinoma (GBC), extrahepatic cholangiocarcinoma (eCCA), and intrahepatic cholangiocarcinoma (iCCA), collected from Cho Ray Hospital and the University Medical Center Ho Chi Minh City. CLDN18.2 expression was assessed by immunohistochemistry using the VENTANA CLDN18 (clone 43-14A) antibody. Positivity was defined as membranous staining of moderate-to-strong intensity (2+/3+) in ≥75% of tumor cells.
Results: The overall prevalence of CLDN18.2 expression was 19.3%. CLDN18.2 positivity was significantly more frequent in eCCA than in iCCA (55.6% vs. 15.1%, p = 0.020). Positivity rates in biopsy and surgical specimens were 23.3% (7/30) and 17.0% (9/53), respectively, with no significant difference (p = 0.566). Among surgical specimens (n=53), CLDN18.2 expression was not significantly associated with perineural invasion, lymphovascular invasion, or nodal metastasis (all p > 0.05).
Conclusions: CLDN18.2 is expressed in approximately one-fifth of BTC cases and is significantly more prevalent in extrahepatic cholangiocarcinoma, supporting further investigation of CLDN18.2-directed therapies in this subgroup. Given the heterogeneous expression pattern of CLDN18.2 and the fact that many patients with advanced-stage BTC lack surgical specimens, biopsy-based assessment remains a clinically relevant reference, although results should be interpreted cautiously and confirmed on surgical tissue when available.
