Abstract
Background: Most surgeons prefer to perform extracorporeal ileocolic anastomosis in laparoscopic right hemicolectomy. The extracorporeal anastomosis can be made by conventional hand-sewn or stapler anastomosis. The study aims to evaluate the results of both anastomoses. Materials and methods: 36 patients performed laparoscopic right hemicolectomy due to right colon cancer with an extracorporeal anastomosis at Hue Central Hospital and Hue University of Medicine and Pharmacy Hospital from December 2020 to September 2022. Results: The mean age was 65.6 ± 14.9; the oldest was 99 years old. The male/female ratio was 0.89/1. The most common reason for hospitalization was abdominal pain, with 77.8%. The high level of CEA before surgery was 36.1%. Cecal tumors accounted for the majority, with 44.4%. The average operative time of the conventional hand-sewn group was 164.7 ± 33.4 minutes, and the stapler anastomosis was 149.3 ± 25.6 minutes (p=0.13). The duration of post-operative intravenous medication for pain control in the conventional hand-sewn group was 4.9 ± 0.7 days, and the stapler anastomosis group was 4.7 ± 0.4 days (p=0.76). The median time of the hand-sewn group was 3.4 ± 2.2 days, and the stapler anastomosis group was 2.9 ± 1.0 days (p=0.19). There were 5.6% surgical site infections in the stapler anastomosis group and 2.8% of anastomosis leakage requiring reoperation in the conventional hand-sewn group. The post-operative hospital stay of the hand-sewn and stapler group was 9.7 ± 7.9 days and 8.7 ± 2.5 days (p=0.6), respectively. There was one mortality related to pneumonia in the hand-sewn anastomosis group. Conclusion: No difference in operative time, intravenous medication time for pain control, post-operative time, or complication rate between anastomosis methods.
