Imaging characteristics and value of computed tomography venography in the diagnosis of May-Thurner syndrome

Abstract

Background: To evaluate the imaging characteristics and value of computed tomography venography (CVT) in the diagnosis of May-Thurner syndrome (MTS) with lower extremity deep vein thrombosis (LEDVT). Materials and Methods: A cross-sectional descriptive study was conducted on 24 patients diagnosed with LEDVT secondary to MTS at Hue University of Medicine and Pharmacy Hospital from January 2021 to April 2024. All patients underwent CTV upon admission. Digital subtraction angiography obtained during endovascular intervention were used as gold standard to confirm the diagnosis of DVT and MTS. Results: the average age was 53.2 ± 11.2 years (range, 22 - 72 years), male/female ratio was 1/7. Common CTV findings of LEDVT secondary to MTS were intraluminal defect (87.5%), increased luminal diameter (83.3%), presence of transpelvic collateral (70.8%), venous wall calcification (29.2%), venous wall thickening (79.2%), surrounding soft tissue edema and infiltration (66.7%). Characteristics of left common iliac vein (LCIV) stenosis on CTV included smallest stenosis diameter: 2.5 ± 0.8mm, stenosis rate: 75.7 ± 8.2%, angulation between LCIV and right common iliac artery 82.2 ± 14.9o, iliac vein angle: 73.6 ± 14.9o. The sensitivity, specificity, positive predictive value, and negative predictive value of CTV were 91.7%, 66.7%, 88%, 75%, respectively. Conclusion: Most of LEDVT secondary to MTS were in acute stage and severe stenosis of the LCIV was well appreciated on CVT. CTV had high value in the diagnosis of LEDVT secondary to MTS.
