Abstract
The Chronic Care Model (CCM) emphasizes empowerment, collaboration, and patient-centered care, but its integration into primary care in Vietnam remains limited.
Objective: To explore the chronic illness care assessment of people with hypertension (HTN) and type 2 diabetes mellitus (T2DM) based on their disease control status and identify associated factors.
Methods: A cross-sectional study was conducted with 213 individuals diagnosed with or T2DM, managed at five primary care facilities in Thua Thien Hue province. Chronic care experience was assessed using the Patient Assessment of Chronic Illness Care (PACIC) scale. Multivariate logistic regression was applied to identify factors associated with PACIC scores in well-controlled and poorly controlled groups.
Results: The proportion of people with well-controlled conditions was 25.8%. The average PACIC score was low, with no significant difference between the well-controlled (2.48 ± 0.68) and poorly controlled groups (2.51 ± 0.75). The aspects of Follow-up and Coordination and Goal Setting and Tailoring had the lowest scores. Among those with well-controlled conditions, better PACIC scores were significantly associated with living with family (OR=202.44; 95% CI=1.2-35016.9) and having a normal BMI level (OR=17.42; 95% CI=1.4-223.7). In the poorly controlled group, higher PACIC scores were linked to living in mountainous areas (OR=17.85; 95% CI=2.2-143.8), not consuming alcohol (OR=2.99; 95% CI=1.1-8.3), and having nine or more medical encounters per year (OR=4.95; 95% CI=1.6-15.5).
Conclusion: Strengthening primary care networks and promoting patient-centered care approaches are essential to improve the quality of chronic illness management.
