Evaluation of CTP - creatinine for predicting prognosis of cirrhosis patients with acute variceal bleeding

Abstract

Background: Variceal bleeding is a severe complicaton of portal hypertension due to cirrhosis with high rate of motality. This study to evaluate the accuracy CTP - Creatinine score for predicting early rebleeding and mortality and in first 6 weeks in cirrhosis patient with acute variceal bleeding. Methods: Prospective study in 118 cirrhosis patients presenting with acute variceal bleeding were hospitalized and diagnosed by upper GI endoscopy. Exclusion criteria were patients with chronic kidney diseases. Results: The mean values of the CTP - Crea I and CTP - Crea II score are 9.03 ± 2.73 and 8.97 ± 2.65, respectively. The CTP - Crea I score was valuable in predicting outcomes early rebleeding with AUC = 0.788, early death with AUC = 0.860 and within the first 6 weeks: rebleeding with AUC = 0.672 and mortality AUC = 0.818. The CTP - Crea II score has predictive value for outcomes early rebleeding with AUC = 0.771, early death with AUC = 0.859 and mortality within the first 6 weeks with AUC = 0.805. Conclusions: CTP - Creatinine scores have good value in predicting outcome in cirrhosis patients with acute variceal bleeding. Can be applied to evaluate and stratify those cirrhosis patients in clinical practice in Vietnam.
