Abstract
Background: Ligation of the intersphincteric fistula tract is described as sphincter-preserving, yet robust evidence on the pre- and postoperative continence function remains scarce. Almost none analysed outcomes separately for healed and unhealed patients.
Objectives: To assess continence and quality of life before and after LIFT procedure for complex anal fistula.
Materials and methods: 144 patients undergoing LIFT procedure for complex anal fistula were followed up at least 12 months. Continence was measured with the Wexner Cleveland Clinic Incontinence Score and quality of life with the Fecal Incontinence Quality of Life scale, both administered by a fixed protocol before surgery and three months afterwards.
Results: Success rate was 83.3% over a median follow-up of 18 months. Median Wexner score was 0 at both assessments (p = 0.052). Quality of life improved significantly in lifestyle, coping/behaviour, and depression/self-perception (all p < 0.01) but not in embarrassment (p = 0.736). Continence remained unchanged after successful surgery (p = 0.655) but deteriorated after failure (p = 0.034), with seven of twenty-four patients developing new impairment, all confined to flatus occurring less than monthly. Patients who failed had poorer baseline scores in all four domains (p < 0.001), yet those who succeeded retained better postoperative lifestyle (p = 0.031) and embarrassment (p = 0.006) scores.
Conclusions: The operation improved disease-relevant quality of life without measurable continence deterioration in patients achieving fistula healing. Functional deterioration within the cohort was exclusively observed in patients with procedure failure and those who underwent sphincter-dividing salvage surgery.
